Getting your heart rate up for cardiovascular health doesn't have to involve high-impact activities that strain your joints. For individuals recovering from injuries or those managing chronic pain, low-impact cardio offers a viable and effective alternative. Experts highlight several options, including the elliptical, stationary bike, and even a modified treadmill, as excellent ways to stay active without excessive stress on the body.

“It’s a great way to get exercise in and stay active without putting a ton of stress on your joints, especially if you're someone that has pre-existing medical conditions or injuries,” explained Dr. Megha Abraham, assistant professor and sports medicine physician at UTHealth Houston. Low-impact cardio emphasizes maintaining intensity while minimizing the jarring forces associated with high-impact exercises.

One expert-recommended method is utilizing an incline treadmill. While running on a flat surface can be hard on the joints, increasing the incline can significantly alter the biomechanics of your stride. “What I like about adding incline is that it changes the position of your ankle, hip, knee, and spine,” said Milica McDowell, PT, MSPT, DPT, associate vice president of education at US Physical Therapy. “For anybody who has difficulty managing load on a flat surface, taking them to an incline is going to feel much more gentle.” This adjustment not only modifies body mechanics for better impact absorption but also inherently reduces the downward force, according to Alex Rothstein, EdD, MS, CSCS, exercise physiologist at New York Institute of Technology. “You’re not really jumping up and landing down on the treadmill; you’re kind of stepping up,” Rothstein noted.

The Elliptical and Swimming Options

The elliptical machine is perhaps the most recognized low-impact cardio option. Originally designed as a gentler alternative to the treadmill, its design keeps your feet in constant contact with the pedals, drastically reducing impact. “The elliptical originally came on the market as a lower-impact alternative to the treadmill,” Dr. McDowell confirmed. Beyond impact reduction, the elliptical engages the upper body, contributing to a higher calorie burn. For optimal use, Dr. McDowell advises seeking machines that allow a natural stride rather than those with a fixed gait, ensuring the movement aligns with your body’s natural motion.

For those seeking the lowest impact possible, swimming emerges as a top choice, though it requires access to a pool. “For anyone that’s dealing with an injury or really bad arthritis or anything like that, I always recommend swimming, water aerobics, and things like that,” Dr. Abraham stated. While swimming involves some exertion, its buoyancy minimizes stress on the joints. However, Rothstein pointed out a limitation: swimming primarily involves concentric muscle contractions and lacks the eccentric contractions crucial for real-world functional strength, suggesting it should ideally be complemented by other forms of fitness.

Stationary cycling, particularly spin bikes, offers another excellent joint-friendly cardio solution. Dr. Abraham highlighted its benefits, especially for those with knee or hip injuries: “It’s very joint friendly, and you can control the intensity, so you can still have your HIIT workouts and things like that.” Using cycling shoes that clip into the pedals enhances the workout's efficiency by enabling a more complete pedal stroke. Rothstein noted that cycling can become an intensely engaging activity for participants.

Even the often-disliked air bike, known by brands like Assault Bike or Airdyne, presents a compelling case for low-impact, high-intensity training. Dr. McDowell described it as combining elements of the elliptical and stationary bike, providing a full-body challenge. “When you think about the bike, you’re taking off some of your body weight, because you’re seated,” she explained. “And with the air bike you’re upgrading the cardio demand and challenging your upper body because of the handlebars.” The air bike's resistance is directly proportional to the user's effort, allowing for scalable intensity.
